Medicare and Medicare Advantage Plan Members
Dr. Grant provides Comprehensive In-Home Medicare Wellness and Risk Assessment Exams around the Coastal Region of Eastern North Carolina.
These exams are covered at no cost to you by your insurance provider and are conducted to identify and meet the medical and non-medical needs of vulnerable seniors to positively impact health outcomes.
This wellness visit will assess individuals holistically and comprehensively in their own environment and, when possible, connect them to needed community resources and care coordination.
